CDE 5-Day Advanced Course in Diabetes Care for Healthcare Professionals

Who Should Attend?

This is an Advanced Course, and is aimed at Health Care Professionals who have a basic knowledge and understanding of diabetes mellitus. It is designed to give an extensive overview of the core principles of modern team diabetes management, so enabling the participants to understand the condition in sufficient depth, to make a real difference in the lives of people with diabetes.

Pre and Post Course multiple choice knowledge evaluation tests are administered, to allow for an evaluation of the learning experience.

Typically, attendees include doctors, nurses, pharmacists, biokineticists, and other healthcare professionals. Pharmaceutical company employees (sometimes even those without a formal healthcare qualification), have also attended this Course.

Attendance of this course is a mandatory contractual requirement for doctors wishing to operate as a CDE-affiliated "Centre of Excellence".

Course Costs

All prices quoted are applicable for 2015, and are in SA Rand, and include VAT. Should you pay in a foreign currency please add on R200 to cover the foreign exchange costs that the bank levies. CDE will not cover the foreign exchange costs.

For Medical Doctors and Specialists:
– Advanced Course Price R9 000.00
– Early Bird price (Book and pay at least 30 days before commencement of course) R7 900.00
For Other Delegates (including allied healthcare practitioners):
– Advanced Course Price R6 600.00
– Early Bird price (Book and pay at least 30 days before commencement of course) R6 200.00

Payments may be made via direct deposit or by a deduction from capitation fees due to the practitioner. A 20% non-refundable deposit is required upon registration. Full payment is due 60 days prior to the course - if not received you will be deleted from the registration list.

Terms & Conditions of Applications
  1. 5-Day Advanced Course in Diabetes fees are charged per person and are inclusive of VAT. The fees include lunches, teas and free, secure parking.
  2. Certified Halaal and Kosher meals are available on request. A surcharge of R300 per person per day for two tea/coffee breaks as well as lunch is applicable. You will be liable for the course fee and R1500 for Halaal or Kosher meals. R7900 + R1500 or R6200 + R1500 or R9000 + R1500 or R6600 + R1500.
  3. The sponsor should obtain a voucher from John Bekker in the event of a sponsorship. The sponsored delegate must register with the voucher number within 7 days. The voucher is valid for 7 days from the date of issue.
  4. ALL costs over and above those covered by the course fee/s totalled for this Application are for the account of the delegate (e.g. flights, accommodation, transport etc.).
  5. Delegates must arrange their own accommodation if applicable.
  6. Places for each course are limited and are allocated strictly on a “First Come, First Served” basis (NO ‘pre-bookings’ or ‘reservations’ are entertained). To secure a place in the course of your choice, early application is recommended.
  7. Allocation of a place on a course of choice is subject to ALL of the following:
    1. Successful completion of the 2015 Application process. To ensure this, please fill in ALL fields completely and provide us with a valid e-mail address to which we can send all confirmations (N.B. We do NOT fax or courier these). Incomplete applications will be rejected;
    2. Acceptance of these Terms and Conditions of Application;
    3. Our e-mail confirmation that we have received your Application and allocated you a place on a course
  8. The attending delegate must read these notes and complete and submit their Application PERSONALLY. We will not be liable for any errors, loss or inconvenience resulting from delegation of this task to someone else or from the non-receipt of all the relevant information contained here.
  9. An administration fee of 10 % of the total due will be levied for requests to change to a later course, if made less than 30 days prior to the commencement of a course.

    • NO refunds or credits will be given to delegates who cancel within 12 days of a course commencing or who do not show for part or
      all of a course;
    • Replacements will be accepted.
  11. The act of completing and submitting your Application Form constitutes a binding agreement acknowledging that the above has been read, understood and accepted.

  2. Places for each Course are limited to a maximum of 84. Early bookings (60-90 Days prior to commencement) are recommended.

Continuing Professional Development (CPD)

The Course offers 34 contact hours. The Wits Health Consortium CPD Office has accredited the Course for 30 CPD points for Medical Practitioners and other Healthcare Practitioners registered with the Health Professions Council of South Africa. CPD certificates will be provided (if applicable).

Official completion certificates are provided to delegates who achieve a mark of at least 60% in the final Post-Course Knowledge Evaluation.

A distinction certificate is provided to delegates who achieve a mark of at least 80%.

Certification and Accreditation as a CDE "Centre of Excellence" Provider

Attendance at this Course, and achieving a pass mark in the final Post-Course Knowledge Evaluation, are pre-requisites for medical doctors wishing to become providers within the CDE "Centres of Excellence" network.

Advice
Dress Code:

The dress code for the duration of the course is comfortable, smart-casual.

Flight Arrangements:

The Course will run from 08h30 to 17h00 daily, except Wednesday and Thursday. Please ensure that you book your return flights (if applicable) from 18h30 onwards (19h30 onwards for international flights). This will give you enough time to complete the Course and get to the OR Tambo International Airport on time. We would appreciate your assistance in this regard, as it is extremely disruptive to have participants asking to write their final test earlier so that they can catch an early flight. This will not be allowed. If you choose to leave early and miss the final written test, you will forfeit your Official Course completion certificate.

Meals:

Morning tea, lunch and afternoon tea is provided to delegates. Dietary preferences are catered for, including: Kosher, Halaal, and vegetarian. Certified Kosher and Halaal meals do attract a surcharge of R300 per person per day - If you choose either of these options, please add this surcharge to the payment for your applicable Course fee. If we are notified of any food allergies, these will be also catered for within reason. The cost will be the Course fee (R6200, R7900, R6600 or R9000) + the Kosher or Halaal meals (R1500) = total.
